she's peeing in the house again.

I was wondering if anyone had any advice for us. She was doing completely fine on her potty training, maybe one accident in the house all of september, and a few times here and there in her crate. But then already into this month she has peed three times in the house. She used to stand by the door when she needed to go, and stopped doing that, but without any accidents. Now all we get is her leaving the room for a second and coming back with her ears down.

Did she forget that she's potty trained?
